Fees & Payment Information

Transparent fee structure — no surprises.

SAP and RMV / court-ordered substance use evaluations are not covered by health insurance.

Full payment is required prior to the scheduled assessment.

Written reports, documentation, or DOT return-to-duty clearance will not be released until all fees are paid in full.

Payment must be made by the time of the appointment and be fully processed before reports are released (3 to 5 business days from time of payment).

SUD Evaluation / Risk & Recidivism Assessment – $400
(Insurance does not cover RMV-required evaluations.)

Missed Appointment / Late Cancellation (under 24 hours) – $100

Initial SAP Evaluation – $500

Reassessment – $375
Required if treatment is not initiated within 30 days or not completed within 90 days

Missed Appointment / Late Cancellation (under 24 hours) – $100

Additional Consultation – $80 per 30 minutes

Secondary Documentation (after 90 days) – $40–$90
(Return verifications, updated reports, additional documentation)

Returned or Invalid Check – $30

Credit Card / Venmo Processing Fee – 4%

Laboratory urine screens or breathalyzer testing, if required, are paid directly to the independent testing facility.
If referred for education or treatment services under a SAP treatment plan, payment and insurance arrangements are the responsibility of the client and the treating provider.
Outstanding balances with outside providers may delay final SAP clearance.

Credit card or Venmo (incur 4% processing fee)

Check or Money Order may be mailed to the practice.
